This is a reminder that vaccine status forms are ​due no later than October 1, which is one week from Friday. The City of Madison has mandated that this form is a condition of employment, so it is critical that this form be completed and returned to us.

Metro Staff,

As you know, in response to the Delta variant of COVID-19, the City of Madison announced that a policy was being rolled out to ensure that all employees are either vaccinated or undergoing weekly testing. Further, yesterday the Biden administration issued an executive order requiring vaccination or weekly testing for all employers with over 100 employees which does apply to us.

Print this form to notify us of your vaccination status. We will also be placing forms in mailboxes and handing them out as needed. ​

This form is mandatory for all employees and must be turned in on or before Friday, October 1. All employees must complete the form, regardless of their vaccination status. Those who are vaccinated should include a copy of their vaccine card or print of their online database record. The online database can be found here: https://www.dhfswir.org/PR/clientSearch.do?language=en.

Forms and documentation should be submitted in one of three ways:

  • Placed into an envelope and dropped in the secure drop box outside dispatch
  • Emailed to Fran ​Taylor at ​ftaylor@cityofmadison.com
  • Delivered to Fran in person

The forms will be handled confidentially within Human Resources. The only information that will be shared with your supervisor is whether or not an employee is required to be scheduled for weekly testing. Details on the procedure for testing will be shared later this month.
