What is this “City of Madison Notice” that I received?
The notice outlines your rights as a patient with regard to the confidentiality of your medical reports and information. We are required by federal law (HIPAA) to provide a copy of this notice to all of our patients.
There is an acknowledgement slip on the last page. If you want to acknowledge receipt of this notice, you may fill it out and send it in; however, you do NOT need to do this; it is up to you.
