About the Well Wisconsin Program:
Reach your health and well-being goals with the Well Wisconsin Program. Through our partnership with StayWell, connect with a health coach or participate in the new online health assessment, digital health programs, and challenges. The new StayWell online platform and mobile app experience is built to inspire healthier habits, healthier minds, and healthier bodies.
$150 Well Wisconsin Incentive
Once you’ve completed your three activities, your gift card will automatically be mailed to the address you have on file with your employer/State of Wisconsin ETF for health insurance enrollment. Watch your email for confirmation that your gift card has been sent. Please allow up to 3 weeks for the gift card to arrive.
Health Assessment: When you take the 15 minute health assessment, you'll learn how everyday choices affect your well-being.
Well-being Activity: Complete one of the well-being activities. You will have the option to choose the activity that most interests you.
Health Check: Choose one of five options to get a status on your health.

BIOMETRIC SCREENING: Go to webmdhealth.com/wellwisconsin and click “Get Started” on the Biometric Screening card. From there, you have two screening options:
At-home test kit
Request a kit by clicking under “Home Test Kit” and ordering on the Quest site.
Health Care Provider Form
Download the fillable form by clicking under “Health Care Provider Form.”
DENTAL CLEANING: Complete a routine dental cleaning visit between October 1, 2020, and October 8, 2021.Report your completion on webmdhealth.com/wellwisconsin by clicking “Rewards” and then, next to Health Check, clicking “more.” Enter your cleaning date and click “save” in the Annual Dental Cleaning activity.
COACHING SESSION: You have two options for coaching interactions:
Phone Option
Complete one call with a WebMD health coach by calling 800-821-6591.
Online Option
Complete a series of online messaging interactions by clicking “Coaching” on webmdhealth.com/wellwisconsin.
The Well Wisconsin incentive program is a voluntary program available to employees, retirees and spouses enrolled in the State of Wisconsin Group Health Insurance Program, excluding Medicare Advantage participants who have incentives available through their health plan. The Well Wisconsin incentive will automatically be issued to eligible participants upon completing the applicable activities. All wellness incentives paid to participants are considered taxable income to the group health plan subscriber and are reported to their employer, who will issue a W2. In some cases, the Wisconsin Retirement System acts as the employer. Retirees, continuants and their spouses will have some taxes withheld from the incentive amount earned.
