Provisional voters who did not have an acceptable form of voter ID at the polls on Election Day have until 4 p.m. to get a copy of their voter ID to the City Clerk's Office. ID may be submitted in person, via e-mail sent to voting@cityofmadison.com, or via fax. City of Madison libraries are able to provide voters with assistance scanning and sending voter ID via e-mail.

Provisional voters who registered to vote on Election Day with proof of address, but who were unable to write their Wisconsin driver license number on the voter registration form, have until 4 p.m. to get their Wisconsin driver license number to the Clerk's Office. The driver license number can be provided in person, through a phone call, via e-mail, or via fax.

The Board of Canvassers will count the provisional ballots of voters who provide missing information to the Clerk's Office by 4 p.m., and will add those votes to the official tally.
